Cancer-related fatigue is one of the most common and frustrating side effects of treatment. It can also feel different from everyday tiredness. It is often heavier, feels foggy (“chemo brain”), more persistent and not always relieved by rest. It may continue during chemotherapy, radiation, hormone therapy and even into survivorship. It may also change depending on where you are during your treatment cycle.
While it may seem counterintuitive, one of the most effective treatments for cancer-related fatigue is exercise.

Research consistently shows that appropriately prescribed exercise reduces cancer-related fatigue, improves mood, supports sleep and helps maintain muscle mass during treatment. Graded exercises support circulation, maintain muscle mass, and provide nervous system regulation. Over time, this builds physical resilience rather than depleting it.
Fatigue is often linked to muscle loss, reduced cardiovascular fitness and changes in routine during treatment. Progressive strength training helps counteract deconditioning. Breath-led movement and Pilates-based work can also help regulate the nervous system and improve energy consistency throughout the day.
Importantly, exercise during cancer treatment is safe when guided appropriately. International oncology guidelines now recommend exercise as a standard part of cancer care because of its impact on fatigue, physical function and overall quality of life.
